Before the incident
The Employee walked into [REDACTED] to peel pack front lite (glass pane) breakage.
What happened
On [REDACTED] at [REDACTED] Peak Employee [REDACTED] was struck by a falling 96 X 130 X 6.0 mm pack at finger bay 614. Based on the video surveillance [REDACTED] dismounted the pack truck then put on a hard hand and face shield. It appears he picked a piece of rope with suction cup. Shortly afterwards he entered the bay 614. After about 20 to 25 seconds in the bay the pack fell unexpectedly striking [REDACTED] in his lower body. At this time we do not know exactly was caused the pack to fall. [REDACTED]’s actions in the bay are mostly out of the camera view
