What happened
The injured employee was operating a pack truck that ran out of fuel. Another pack truck drove to the area to assist. Neither truck had glass being transported. Gas cylinders were swapped from one truck to the other. As the assisting driver attempted to drive away their mast hit the mast of the down truck and pushed the truck over the injured employee 's left foot as they were installing the cylinder.
