Before the incident
Associate had picked a case up off the floor and placed it on a pallet stack and began to walk backwards.
What happened
Associate tripped over a pallet on the floor thus injuring her lower back and left shoulder.
Injury or illness
Associate walked backwards to give space to Associate B operating the clamp. Associate A didn 't see a pallet on the floor behind her resulting in her tripping and falling backwards. Associate A reported the incident to a supervisor and that she was experiencing some discomfort in her back however did not think it was severe enough to get it looked at and signed the medical refusal form.
