Before the incident
Associate was trouble shooting why the hood of the Lohia [REDACTED] machine was not returning to the lower position. Associates foot was simultaneously resting on the foot pedal while she was attempting to loosen the strap on from the anchor point.
What happened
Tension on the super sack strap pinched the associates finger to the anchor point. In response to having their finger pinched the associate forcefully yanked their hand away which caused the tip of their finger tip in the approximate area of the cuticle and distal digital pulp to become amputated.

Associate 's finger got caught between the SSK strap and the hook which caused the very tip of her finger tip to become amputated.
