What happened
assisting a resident in [REDACTED] with the restroom. the pt grabbed the bar and stood up when he was done using the restroom the pt lost their strength and went to fall. I caught the resident but could not get the pt back onto the toilet causing my to stretch out to grab the commode for the pt to sit on. after i sat down to do paperwork I was called to fix a bed I felt terrible pain in my upper back between the shoulder blades and in my back lower right side. reported via phone by [REDACTED] with [REDACTED] was filled out day of incident but origami did not come over.
