What happened
EE c o multi insect bites while in USX TRKEE states upon being assigned a nearly brand new [REDACTED] Volvo truck she observed what she believed was mold around the side cab window. Upon further inspection she observed that the truck was infested with bugs. EE complains of bites all over her body from head to toe. EE told her fleet manager sometime around 6 3 25 but a claim was not reported. The EE sought treatment one time on [REDACTED] 25 at HomeTown Urgent Care [REDACTED]. [REDACTED]. EE requesting bill for 251.25 be covered under WC policy.
