What happened
On [REDACTED] at approximately [REDACTED] an associate climbed a 10- to 12-foot A-frame ladder to remove the sanitation tubing from Line 2 's filler. While standing on the top step of the ladder with one foot on the filler tubing (which is U-shaped) they leaned forward and stretched at the waist to remove the tubing. This caused the associate to lose his footing and hyperextend his foot ankle.
