What happened

Teammate reported that he noticed discomfort in his left shoulder neck trapezius between [REDACTED]. As the morning progressed the pain became more pronounced. TM went to break at [REDACTED] still noticed pain. When he stood up and grabbed his lunch box he noticed much more significant pain so he notified [REDACTED]. [REDACTED] instructed the TM to notify [REDACTED] the plant manager. [REDACTED] instructed the TM to go to Alabama Specialty Clinic. TM reported he used sit down lift truck to retrieve a pallet of TC bolts. The TC bolts were stored in cans kegs. TM drove the lift to the work table 15 in front of gametime. TM raised pallet up to get cans kegs at good ergonomic height. His order was pick 3.5 cans kegs of TC bolts. TM leaned the can keg over with his right arm to dump TC bolts into a 2-gallon bucket he was holding with his left hand arm. TM filled up the 2-gallon bucket full then used both arms to lift the bucket to shoulder height (hands at should height on the bucket handle
