What happened
Fracture to proximal end of great toe left foot EE was operating a Walk-Behind Motorized Pallet Jack to move totes of finished goods into the staging area. Once task was complete EE went to park the pallet jack in the designated area. EE released the controls prior to pallet jack coming to a complete stop. Pallet jack rolled onto left foot causing the safety toe in EE 's shoe to flex against his foot. No Serious Injury. EE followed up with Occ [REDACTED] 13 25. X-Ray Indicates Fracture to proximal end of great toe left foot.
