In the process of discontinuing a patient 's dialysis treatment I was pulling tape off of his needle site and the needle was hanging half ways out. When I went to pull his needle after i put gauze over the site to control the bleeding the needle pulled out on it 's own before I could pull and activate safety device.

as I was in the process of pulling his needle out it came out on its own.
