On [REDACTED] at approximately [REDACTED] wrapper operator went over to the Mod 2 stacker area to clear a jam-up when she slipped on a waffle that was on the floor. EE slipped and fell on her back. After the incident EE declined any type of First Aid or being connect to the nurse line. On [REDACTED] employee called the nurse line and got recommendation to take over-the-counter medicine.
