What happened
On [REDACTED] at approximately at [REDACTED] Ops operator was cleaning cooling belt when he slipped from the second step from the top of rolling ladder and fell on his back butt five steps down. Employee received cold treatment and later was connected to the nurse line. The nurse line recommended to applied ice OTC medication and to go to a Concentra clinic. Employee did not go to the clinic in the day of the incident as he preferred to sleep and rest. On [REDACTED] EE went to the clinic he did not get any PT session no work restrictions but he received prescription medication. After clinic appointment Concentra released to full duty.
