On [REDACTED] sanitation employee was lifting floor mats onto a sanitation cart when she felt back pain but EE did not reported on date of injury. She reported on 5 1 nurseline sent her to hospital and she received prescription medication (naproxyn 500mg and acetaminophen 1000mg) and x rays. Hospital provided a note to be off from work which caused one day of lost time. On 5 6 employee went to Concentra to be checked and she was released from care.
