Operator was watching part face via window on closed QT door.
What happened
Running 123226 (approximately 49 pounds) in the [REDACTED]-250. While making the first facing cut on [REDACTED] when there was 1 distance to go traveling towards the center of the part with the RPM 's around [REDACTED] the part came came out of the chuck. It ejected upwards and slammed into the door. The door came off of the machine and slammed into the employee. This caused the employee to be pushed backwards into the work bench which moved around 10 inches back. When the door made contact with employee it caused a V shaped impact laceration with each leg being roughly 1 long.
