Before the incident

On [REDACTED] at approximately [REDACTED] hours I was adjusting my workspace to transition from a seated to a standing position using my monitor stand which allows for height adjustments. As part of this process I needed to retrieve my padded mat from under my desk for ergonomic support while standing. To do so I pushed my rolling chair back to clear space. Unbeknownst to me a loose string from the worn and ripped carpet under my desk became entangled in one of the chair 's wheels. When I took a step forward my right foot caught on the obstruction causing me to trip and fall. As I lost my balance I landed knee-first on the ground hitting my right elbow on the chair before making full contact with the floor. No machinery chemicals or fumes were involved in this incident.
