Walking to television above time clock from office to plant

On [REDACTED] EE was walking out into the aisle leading to the plant floor to check the communication television by the timeclock before leaving for the day When EE walked out of the office door leading to the plant EE foot hit an absorbent pad that had been placed on the floor upon investigation it was learned that an individual had put the pads there to absorb coffee that someone had spilled on the floor [REDACTED] minutes prior to the injured employee coming in contact with the pad The individual intended to come right back and finish cleaning up the spill but did not prior to EE falling EE foot slipped on the pad and EE fell EE was unable to move and stated EE could not move ankle An ambulance was called and EE was transported to ACMC X rays were negative and EE was diagnosed with a left ankle and knee sprain and released back to full duty
