What happened
Employee adjusted the settings for the edge deletion wheel to “[REDACTED]” rather than homing the wheel manually. The [REDACTED] was set for every 5th sheet of glass. The employee then went to back feed a sheet of glass and didn’t realize the sheet they were back feeding was the 5th sheet in the run so as they were centering aligning the sheet to be even on the table the edge deletion wheel “homed” and fractured the employee 's left hand.
