The incident occurred on the southwest corner of the main bay department. The employee was standing on the west side of the tunnel as it was being rotated.

The employee was helping with the tunnel roll. He was standing next to the tunnel. He looked away for a moment. When he looked away the squaring bar came loose and hit him on the head.

Squaring Brace was not tightened against the tunnel wall. As the bridge was being rolled the employee looked away. As he looked away one of the braces came loose and fell and hit the employee on his head and then scraped down his right torso.

Nature of Injury Illness:: Bruise Contusion; Body Part(s):: Head

Squaring or bracing bars are used to ensure that the tunnel while it is being welded. The bars are inserted on the inside of the bridge. They are then tightened in place in an X shape. One was tightened to ensure the wall was square. The other was put in place but was not as tight as the first. Tension on the second squaring bar was loose thus allowing the bar to break loose and fall and hit the employee.
