At approximately [REDACTED] on assignment 5096 [REDACTED] states that he injured his hand while he was selecting cases of soda in aisle G05 He states that there was plastic wrap on the floor near the slot he was selecting and after grabbing the soda case his foot became entangled in the plastic causing him to trip He fell down to the ground where his right hand struck one of the pallets on his jack and then through a gap between two boards He continued selecting and did not report the incident that nightFrom the description of the item and the approximate time [REDACTED] gave the slot at which the injury occurred was likely G0518101 where he picked 12 cases of ginger ale and had 6 minutes still timeLuis reported the injury to me at the pre shift meeting [REDACTED] showing me the hematoma on his right hand
