Associate reported that she slipped in a puddle of water from an AC unit Upon investigating the fall I observed water that had puddled from the AC unit inside the Plant 41 [REDACTED] that resulted in the associate slipping into the table The associate was sitting in a chair inside the [REDACTED] at breaktime and when she stood up her foot slipped in the water which caused her to hit her head on the table The associates right eye shows some redness with a small knot starting to form The associate also stated that the impact from the fall caused her back to hurt I applied an ice pack to the affected area just above the right eye Maintenance was called to mop the water and a wet floor sign was put in the area to notify other associates The associate and I had a discussion about always paying attention to the surroundings She stated that she did not see the water until she stood up
