Before the incident
On [REDACTED] a 2nd shift employee had finished painting a large GPHE carry bar and had moved it by overhead crane to C line holding area. He had placed the carry bar vertically on three 4” x 4” wooden supports unhooked the chains from the carry bar and raised the crane. While raising the crane and chains up the carry bar fell over pinning the employee’s leg between the bar and the floor. The employee called for help and a nearby employee went to him for assistance. The second employee lowered the crane reconnected the chains to the carry bar and lifted it off the injured employee’s leg. The injured employee moved himself out of harm’s way and received first aid from site employees until the ambulance crew arrived on scene. The employee was transported to the hospital for further medical attention.
