Before the incident
Employee was training on manual de-liner machines. They had just completed de-riveting one used brake liner from the brake shoe when they attempted to remove the used liner and throw it in the waste bin.
What happened
Employee left foot in actuator pedal housing and shifted their weight inadvertently actuating the press pin which contacted the metal brake shoe. Employee had their RH thumb between the metal brake shoe and used liner they were removing which was smashed when the press pin pressed into the brake shoe metal.
