Walking in patient room 314 to perform a discharge clean was performing hand hygiene before entering the room.

What happened

[REDACTED] walked into the room and slipped on the floor due to it being wet. There was a large puddle of water that had been a significant amount of water on the floor A wet floor sign was but laid on the side of the wall. The team members pants ripped as she fell forward. [REDACTED] was assisted by another team member [REDACTED]. [REDACTED] began to clean the area up with a mop and place a wet floor sign in a more visible area to alert future staff.[REDACTED] left the room to call the EVS Lead at [REDACTED] with no answer and then called the EVS Supervisors desk phone with no answer. [REDACTED] then came down to the EVS office to speak with EVS Leadership and spoke to the EVS Lead [REDACTED].[REDACTED] was asked to stay on sit to speak with the EVS Supervisor but she had a prior commitment that required her attention outside of work.
