Pulling a bed

[REDACTED] was pulling the head of the bed while walking backwards and coworker was pushing the end of the bed. The bed was turned in the opposite direction that usual. The in-patient bed was unoccupied and being returned to the floor. The steering mechanism was missing the protective latch leaving an exposed sharp metal edge. [REDACTED] lost her footing as she turned to make sure the area behind her was clear. The back of her right shoe became wedged under the wheel so to try and catch herself she tried to step forward with her left foot. The bed was moving faster than she could compensate for at the time and she lost her footing. [REDACTED] was completely walking forward making sure the hallway was clear. The metal mechanism scraped the back of her heal leg leaving a 4 inch scrape and her left foot was then pulled under the front of the bed causing her to fall forward. She doesn 't recall exactly how she landed but can remember sitting up to look at her left foot immediately. Both sh
