employee was looking at grinder to investigate contamination in regrind from machine

plant manager was looking into grinder while employee on other side was also looking. mechanical arm started to come down pinching employee on opposite side. Plant manager saw mechanical arm slowing going down and attempted to take pressure off mechanical arm to allow caught employee to get out of machine. Once other employee was free plant manager let go of piece he was holding up and it came down quickly catching his left forearm and pinching it.
