Employee was trying to reposition move concrete system hoses that were placed on a deck with plywood with aluma beams under the plywood.
What happened
Employee was trying to reposition move concrete system hoses that were placed on a deck with plywood with aluma beams under the plywood. The GC had removed a blue panel (guardrail system) & placed a red barricade tape in place close to the edge. When the employee crossed through the red tape he stepped on the plywood which overhung past the aluma beam which caused the plywood to kick upward behind him causing him to fall to the slab below of about two feet high to a standing position. When he tried to catch his fall he grabbed onto a guard rail post with his left arm causing awkward twisting
