Before the incident
I was working in the clinic. Provider asked for casting supplies so had to go down hall to ER. There are rugs on the floor by doorway and I caught my toe on the rug. No ripple in the rug nothing wrong with the rug. I just had mind on other things as was going to get the supplies. Was not running.
What happened
Toe of shoe caught on rug and I tripped and fell forward onto knees then flat down on the floor. Elbows and knees bumped floor and chest bumped floor as well.
