Guiding the material handler
What happened
FTE was working on the upper deck in the Roasting department. The employee was tasked with being a guide for the material handler while placing a pallet of raw materials on to the upper deck. While placing the pallet onto the platform a piece of the pallet board broke seconds prior to the pallet being placed down. This piece of pallet board was wedged itself under the pallet causing the pallet to fall forward. Several cases fell from the top of the pallet. These cases made contact with the employee 's harness cable causing the employee to fall to the floor.
