Before the incident
A step ladder is commonly used by operators when needing to reach into the little pop bin. When the level in the bin gets low the operators need to move the wrapped little pop candies down toward the bottom opening.
What happened
Employee climbed the step ladder moved the little pops inside the bin toward the opening and then began to descend the step ladder. As was stepping backward from the second rung to the first the ladder became unbalanced with the shift in weight and tipped back. When the ladder tipped employee fell back onto the floor on left shoulder.
