Associate was loading a trailer
What happened
Associate did not report an unsafe work condition. Associate adjusted the lift gate and continued to work however the associate did not notice that the dock plate and loading floor became unsafe and the associate collided with the loading floor when loading the next pallet on the trailer. This caused the equipment to come to a complete stop and sent the associate into the throttle arm causing the injury reported.
