EMPLOYEE (ONE) WAS WORKING ON THE SIDE OF A TANK. ANOTHER EMPLOYEE (TWO) WAS REMOVING RIGGING STRAPS & SHACKLES. AS EMPLOYEE ONE BENT DOWN EMPLOYEE TWO KNOCKED A STRAP & SHACKLE OFF THE TANK WHICH STRUCK EMPLOYEE ONE IN THE HEAD.
