TM was prepping pallets. The stack fell onto [REDACTED] striking him the low back right wrist.
What happened
Lift operator placed on the floor an 18 high stack of pallets behind [REDACTED] to place on the prep line. The lift op placed the stack on top of 4-5 pallets that were already on the floor. One of the 4-5 pallets had 2 missing blocks which made the stack unstable. When the lift op removed his forks from the stack the stack fell onto [REDACTED] striking him in the low back right wrist and forearm.
