Before the incident
During a washdown task on a flour duster at the production line the operator intended to remove a coagulated ball of flour that had been created due to a mix of flour and water. The operator stated that they failed to shut down and LOTO the equipment. The fingertip of their left hand ring finger was caught in the roller which resulted in an avulsion to the tip of the finger including removal of the nail. The employee was seen at the emergency room for medical evaluation and it was determined that surgery was required. During surgery approximately 1 cm of bone from the fingertip was removed resulting in an amputation classification.
