What happened
We were installing the pins into the pivot lugs of the tipper deck. The pin was not fitting into is location so we needed to clean out the hole for the pin to fit. the pin was resting on top of the ladder. employee was walking by the ladder when the pin rolled off the ladder hitting him in the hip. This caused employee to lose his balance and fall to the ground.
