What happened
The employee wanted to show his shovel man a particular area to reach with the pressure washer and proceeded to physically point to its location by placing his arm in between the hopper and the chamfer. Unknowingly the top gantry operator turned on the hydraulic to move the extruder to the middle pit to begin the washout. In doing so the chamfer began to move slowly up pinning the operator 's right wrist
