What happened

When he approached the location the material that he was looking for was in the back of the location and could not reach it with a forklift. The employee set his forks down on the bundle that was in front of the needed material. Then got off the lift and attempted to lift one end by hand and set it down on his forks. When he went to push the pcs forward he lost his footing and fell on top of the pcs he was lifting causing a large laceration to his left forearm
