The employee had just set down a full stack of CHEP pallets and then started to drive around the stack and quickly turning to the left to enter the other side of the pallet stack. The driver was then going to put them into the pallet straightener.

As the employee began to drive around the stack of pallets the driver quickly turned to the left. The rear counterweight of the forklift then swung out quickly to the right striking a guard rail post. This resulted in a vehicle impact shut down event. Upon impact with the guard rail the employee 's momentum continued to the right striking his right eyebrow against a 2-pronged metal bracket. The bracket was positioned eye-level and was mounted to the right upright overhead cage bar.

The employee sustained a 3 4 inch long horizontal laceration along the right eyebrow which resulted in three sutures.
