Before the incident

The employee was working in the picking mod on the fourth floor and was required to use the Vertical reciprocating conveyor (VRC) to send their cart to a different floor.

What happened

On [REDACTED] at approximately [REDACTED] athlete was picking for 3pl department and RF scanner sent that athlete to the fourth floor. After picking on the fourth-floor athlete was sent to a different floor to continue their task. Athlete then waited for the [REDACTED] to relocate to a different floor. When [REDACTED] made it to the floor athlete opened the door and lifted the bar to place their cart on the [REDACTED]. Upon opening the [REDACTED] door athlete lifted the security bar but did not push the bar all the way up and when they entered the [REDACTED] to place the cart on the bar fell and hit the athlete on the back of the right side of their head. Another athlete witnessed the incident and immediately asked if they were okay and athlete stated yes. After the incident at approximately [REDACTED] athlete went and found a Lead that could translate ASL due to the athlete being deaf.
