What happened
On [REDACTED] CCA was moving a pallet with the electric pallet jack from the vestibule to the bundled flat staging location. Carrier was attempting to get the pallet through the double doors when she misjudged the speed of the pallet jack when she stopped at the door and the main wheel of the pallet jack ran over the topside or her right foot. Carrier attempted to continue through the morning but began to become uncomfortable with the pressure from her foot and sought medical attention.
