What happened

Patient was assisted with incontinence care. two person assisted to turn. The patient had been medicated prior to turn. She has significant pain. The patient grabbed my arm and her nails punctured and scratched my left forearm. There was a small amount of bleeding from four separate spots. There is a little swelling and discoloration. The area affected approximately 2 x 2 area distal to the AC of the left arm. Wound was cleansed and dressed with appropriate adhesive dressing.
