What happened
EE was in process of monitoring a confined space entry. There is a two-tear platform at the entry point of the confined space. The EE was standing on the top portion of the platform (5-inch depth) to get a better view of the entrants while material was being removed from the tank. EE lost footing rolling ankle onto the lower platform (15-inch depth) and continued to fall to the ground. Fire brigade responded iced ankle and helped EE to personal car where a family member brought him for medical check-up.
