A Packaging Machine Operator Entered The Jmp Robot Automated Palletizer Area After Engaging The Known Interlock She Reached Into The Back Of The Accumulator To Grab A Case That Did Not Make It All Of The Way Off The Rollers From The Packaging Machine And Into The Accumulator Area She Reached In From The Opening At The Front Plate Of The Accumulator And Grabbed The Box She Pulled The Box Off The Rollers And Into The Accumulator Completely As Soon As She Did The Air Actuated Lifter Moved Up And Pinned Her Left Arm Causing A Sprain
