EE was preparing the machine to run the material.
What happened
The carriage saw on the finger joint machine the EE was working on was not cutting correctly so he pulled off the safety grate and went to check tension on the drive poly chain. the EE didn 't realize that the key for the sprocket had fallen out and the pulley wasn 't moving but the shaft of the motor was still turning and when he put tension on the poly chain it was enough to cause the pulley to start rotation again and it pulled his left index finger around the pulley. LOTO procedure was not followed.
