EE got off the forklift to assist a Press Operator in removing the punch plate (the die) from the Cincinnati Punch Press.

EE had got off the forklift to assist a Press Operator in removing the punch plate (the die). EE pulled up as close to the machine as they could without hitting the press. The punch plate was suctioned to the channel on the machine and when the plate came out it came up and slipped off the edge of the machine and missed the forks on the lift causing the plate to fall and lacerate EE 's left upper leg. EE was transported to Goshen Hospital via ambulance. 29 sutures were needed to close the wound. EE was taken off work for 65 days and upon their return EE was on restriction for 20 days.
