Before the incident
Cleaning the area of work and organizing the wheels on the conveyor belt.
What happened
Worker shifted a wheel on the conveyor from forwards to backwards and that triggered the sensor in the washing machine. Which then automatically pushed a part out and bumped another part that caused a chain reaction in movement. That ended with one of the last two wheels to be moved and causing one to fall. Employee rushed to the falling wheel to catch it and in the process injured their hand.
