What happened
[REDACTED] was operating a boom lift to reach the level 2 of the east tower balconies to cut in chamfer. While maneuvering his boom lift into place he took it upon himself to boom up and over handrail that was in place around the perimeter. This created pinch point with himself and the soffit of the balcony. IE then stuck his head out of the lift to see if he was going to the clear the handrail. At this time [REDACTED] wanted to move his lift forward. When he went to move forward with his head outside the basket he used the wrong joystick. Instead of the joystick to drive forward he used the joystick to boom the lift up. This caused his head to be caught between the soffit and the handrail of the lift.
