What happened
on [REDACTED] - TM reported that she was going to have a nerve study completed for an on going pain issue in her left and right wrists. My office had no record of her having pain from and work related incidents. I advised her to continue with her personal treatments as she had been doing and we completed the documentation for Occu-sure to begin an investigation. On [REDACTED] she presented a RTW form with restrictions of No Repetitive Motion to Bi-Lateral Hand nor exposure to vibratory tubes. We attempted to accommodate these restrictions but The TM stated she could not perform the tasks due to pain and was placed on LOA. Occu-Sure completed their investigation and accepted the case as Work Comp on [REDACTED]
