Before the incident
On [REDACTED] at [REDACTED] I was heading to the HR Office and was approached by the employee she telling me that she had had an accident I told her to explain. The employee told me that she was working on line 1 cleaning the floor near the tray storage she was using a mop backet and when she tried to wring out the mop the arm of the mop backet hit her hard in the left breast. I asked her if she needed medical attention and she told me no but that she was in pain. I took the employee to the HR office and asked her again if she needed medical attention and she said yes. I went with the employee to the area where the accident occurred and I told her to show me where it happened and the tools she was using. I took the mop backet to my office and inspected it with my supervisor we did tests and it was working normally then we checked it in detail and found that it had a broken part. Photos were taken of it and the mop backet is out of circulation and we isolated for investigation.
