Before the incident
Caring for a patient at the head of the bed.
What happened
The employee was sitting at the head on the bed working with a patient. While the employee was at the head of the bed a nurse was discarding a syringe and needle that had been used to give a medication to the patient. After giving the medication the nurse placed the used device in the sharps container. The syringe and needle was discarded in a designated sharps container but the nurse that placed it in the container did not realizes that the trap door that drops the sharp into the container had not fully engaged to clear the item. This allowed the needle and syringe to sit into the opening and once the nurse let go of the automatically closing lip the syringe flipped back out of the opening and fell on the employee resulting in a needle stick to the right shoulder. The employee was not the original user of the device but the employee was at the head of the bed tending to the patient and was in the pathway of the falling needle.
