Employee arrived while a product elevator was being reassembled to be placed back in service. Employee elected to help the team reassembling the product elevator.

legs of the product elevator were fully raised into position using a forklift to hold them in the open position but were left unsecured to the forklift. Employee was holding cross-members in place to allow another employee to hammer the pins into place using a dead blow hammer. Injured employee 's hand was struck inadvertently with the dead blow hammer; as a result the injured employee pulled their hand back. the injured employee 's other hand was in the line of fire of the product elevator legs - on the next strike of the dead blow hammer the product elevator legs collapsed down on the injured associate 's hand amputating the 4th and 5th fingers of the left hand.
